Outpatient Appointment
This page includes information about my charges, if you do not have private medical insurance but someone else will be paying your bill, you may wish to highlight this to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charge which they do not pay.
Appointment
Please arrive 10 minutes before your appointment time in order to complete some paperwork. On the day of your appointment please report to the main reception. If this is an initial appointment you will need the name and address of your GP and/or referring specialist and medical insurance details (membership number and authorisation number). You will be asked to complete some paperwork and will then be directed to my consulting room. I will try my very best to see you on time for your appointment. However, some consultations may take longer than anticipated or I may get emergency cases. This could potentially lead to delays in appointments. It is now a requirement that patients provide their NHS number (unless the patient is from overseas). Please ensure this is forwarded to my PA (contact@florian-bast.com) prior to your appointment.
FEES
Insured Patients:
My fees are in line with insurance company guidelines, therefore they are usually covered in full with or without an excess to pay, depending on your policy. It is the patient’s responsibility to obtain an authorisation number from their insurer prior to the consultation. If the insurance company fails or refuses to settle an invoice, the patient is responsible for any outstanding fees.
*{Location} will charge the patient a separate fee for the use of equipment (approx. £270). This fee is in addition to the above fees.
*For insured patients, this fee will be submitted to the insurance company (the patient will be liable for any shortfall).
*For self-funding patients this fee is to be paid on the day of your consultation.
Following your consultation you may need certain tests (such as blood tests or imaging, for example, an X-ray, MRI or CT scan) to help me diagnose your condition. If the test is undertaken by the clinic or hospital, and not by me, the fees for those tests will be determined by the clinic or hospital and charged to you, or your private medical insurer, separately.
If there are any fees which I will charge in relation to any of the tests I advise that you have, I will let you know what those will be.
Prescriptions
Repeat prescription when not part of a consultation: £20
Forms
Completion of forms (excluding medical insurance claim forms): £20
Payment
In the absence of a prior written agreement to the contrary, payment is required on receipt of the invoice. Failure to make payment within 28 days may result in the matter being referred to our debt collection agents whose charges will be added to and payable with the invoice.
Appointment Cancellation Policy
You can cancel your appointment at any time by telephoning or sending an email to the Practice. You must notify the Practice at least 24 hours (1 working day) before your appointment. This allows us to run the clinic efficiently so someone else may take your appointment if you need to cancel. You do not have to give a reason for cancellation. There may be times when appointments are cancelled by the Practice, if this happens no fee will be charged and we will always give you as much notice as possible of cancellation.
If you miss your appointment and have failed to notify the Practice 1 working day before your appointment, you will be charged an administration fee of £50.

Private Medical Insurance
If you have private medical insurance, please contact your insurer before your consultation, to check the terms of your policy, particularly the level and type of outpatient cover you have, including any reimbursement limits on individual consultation fees.

Responsibility for Payment of Invoices
